A preliminary autopsy report has been released for former 'Glee' actor Cory Monteith, who died on July 13 at the age of 31. Sadly, it seems a combination of alcohol and heroin led to his death.

This according to the British Columbia Coroners Service (see official announcement above), which said based on the toxicological analysis, Cory's death was determined to be "a sad and tragic accident."

Monteith, who'd struggled with substance abuse issues since he was a teenager, just completed a rehab program back in April, getting public support from girlfriend and 'Glee' co-star Lea Michele.

His family was notified of the autopsy results before the information was released to the public, and the coroner's investigation is currently ongoing. No further information will be released until it's complete.
