What The Heck Is This Diagram That Fish Drew Supposed To Mean? [NSFW]

We were talking about outie belly buttons today when Connie told a story about her friend who had an outie belly button that was almost 2 inches long!  What!?  Her friend had to eventually get it removed later in life because it was so large.

That moved the conversation over into a weird place.  We compared that belly button to when you get a hernia and you have that sore lump that sticks out of your side.  Well, Fish was trying to make a joke about a prostate exam and making things stick out of your abdomen, but Connie and Steve didn't get the joke (they still don't).  So Fish, decided to draw a diagram ("diaphragm" as Fish would say) to explain, but that didn't help at all.  What the heck is this a diagram of and what does it mean!?
